Request to Issue Employment Contract is required after the Request for Personnel form is completed and approved. This indicates that a candidate has been selected to be hired and ready to issue a contract for employment. Supervisors fill form informing agreed pay, budget account information, supervisor information, date of hire, and job details. Form is to signed by supervisor and Department Head. It will then be approved by CFO and HR.
